Current trends in organizing the care for patients with acquired heart valve disease

Keywords: valvular heart disease, patient phenotype, organization of care, valvular cardiac team

Abstract

Despite advances in research of heart valve disease pathophysiology and active introduction of miniinvasive correction of this disease, heart valve diseases are still poorly diagnosed pathology. These diseases are detected in delayed stages, and there is still inequality in diagnostic and treatment technologies in different regions of the world and our country. In this review, we analyzed
epidemiological data on various heart valve diseases, modern phenotype of these patients and factors associated with changes
of phenotype. The main positions and limitations in organizing the medical care for these patients are presented. A particular attention is focused on importance of “valve cardiac team” making decisions on management in complex and controversial cases,
as well as in comorbid patients with heart valve disease

Barbarash Olga, Research Institute for Complex Issues of Cardiovascular Diseases, Kemerovo, Russia

Federal State Budgetary Institution "Scientific Research Institute of Complex Problems of Cardiovascular Diseases", Director, Professor, Doctor of Medical Sciences, Academician of the Russian Academy of Sciences. In 1984, she graduated from the Faculty of Medicine of the Kemerovo State Medical Institute. She defended her PhD thesis in 1989 and her doctoral thesis in 1996. Since 1997, she has been the head of the Department of Internal Diseases and Clinical Immunology, and since 2007 - the Head of the Department of Cardiology and Cardiovascular Surgery at KemSMU, since 2008 - the head of the Department of Multifocal Atherosclerosis at the Ural Research Institute of the CPSS SB of the Russian Academy of Medical Sciences. Since December 2011, he has been the Director of the KPSSZ Research Institute. O. L. Barbarash is a leading scientist who studies the clinical features and the contribution of various factors to the mechanism of formation of multifocal lesions in atherosclerosis; the development of an effective program for its secondary prevention. Her scientific works are recognized both in Russia and abroad. Within the framework of the priority area of science and technology development in Russia, "Life Sciences", under the leadership of Olga Leonidovna, fundamental scientific research is being conducted in the following areas: the study of molecular, organismic and population patterns of the formation of multifocal atherosclerotic lesions; the creation of new biomaterials for cardiovascular surgery, their experimental and clinical approbation; scientific substantiation and development of promising models for the provision of medical care for diseases of the circulatory system in an industrial region. The results of these scientific studies are the basis for developing a strategy to reduce the negative impact of social change on people's health. In the coming years, these scientific results will make a significant contribution to the study of the mutual effects of social, family, individual and genetically determined diseases, which will affect the improvement of the prevention of cardiovascular diseases (CVD) in Russia and around the world.
