Atrial cardiopathy. Diagnosis of causes, treatment of complications and surgical correction of cardiac remodeling

Abstract

The relationship between atrial fibrillation and thromboembolic syndrome and, above all, ischemic stroke is well known. Atrial
cardiomyopathy is important for understanding the mechanisms of thrombogenicity. In this report, we consider diagnostic algorithm, surgical treatment of a patient with long-standing persistent atrial fibrillation, thromboembolic complications, severe mitral insufficiency and atriomegaly following myocarditis. Structural and functional cardiac remodeling with subsequent arrhythmias and heart failure is a serious problem in modern cardiology and cardiac surgery. Extensive cardiac surgery is extremely difficult and does not always lead to significant clinical improvement. However, quality of life is quite low without surgical treatment
in such patients and associated with high risk of adverse cardiovascular events.

Fedotkina Julia Alexandrovna, Chazov National Medical Research Center of Cardiology

Researcher at the Department of Clinical Problems of Atherothrombosis, Candidate of Medical Sciences In 1997, she graduated from the Pirogov Russian State Medical University with a degree in medicine, and in 1999, she completed her residency in cardiology at the Russian Ministry of Health's RCNPC. In 2002, she defended her PhD thesis. Since 2002, he has been working as a researcher at the Department of Clinical Problems of Atherothrombosis. The main area of scientific activity is modern aspects of multicomponent antithrombotic therapy in various categories of patients, as well as the prevention of cancer-associated thrombosis. He is the author and co-author of 35 scientific papers, including 2 monographs, 1 manual, and recommendations of the Eurasian Society of Cardiology on the diagnosis, prevention, and treatment of cardiovascular complications in antitumor therapy. The Hirsch index in the RSCI is 7.

From 1993 to 1995, he studied in a clinical residency in cardiology at the Cardiology Research Center of the Russian Academy of Medical Sciences. In 2013, he defended his thesis for the degree of Doctor of Medical Sciences. Since 2014, he has been a leading researcher at the Department of Clinical Problems of Atherothrombosis. For many years, he has been responsible for the scientific topics of the Department. He has more than 120 publications in Russian and international journals. Member of the working group of the Society of Specialists in Emergency Cardiology, scientific editor of the journal Cardiological Bulletin. He is the Deputy Chairman of the Ethical Committee of Clinical Trials of the Federal State Budgetary Institution "NMICC named after Academician E.I. Chazov" of the Ministry of Health of the Russian Federation. The Hirsch index in RSCI is 16, in Web of Science and Scopus – 6.

Makeev Maxim Igorevich, Chazov National Medical Research Center of Cardiology

Ultrasound Diagnostics Doctor of the Ultrasound Research Methods Department In 2011, he graduated from the Faculty of Medicine and Biology of the Pirogov Russian State Medical University with a degree in biophysics. In 2013, he completed his clinical residency in ultrasound diagnostics at the Russian Ministry of Health. In July 2019, the Transcatheter Mitral Valve Repair Multidisciplinary Team Training took place (Brussels, Belgium). The main area of scientific and practical activity is the diagnosis of structural heart diseases using transthoracic and transesophageal echocardiography. He is proficient in modern echocardiographic technologies: three-dimensional mode, speckle-tracking echocardiography, noninvasive assessment of myocardial function, and others. Selects patients for transcatheter and cardiac surgery. Performs intraoperative support of various interventional interventions: transcatheter edge-to-edge plastic surgery of the mitral and tricuspid valves, transcatheter prosthetics of the aortic valve, closure of septal defects, prosthetic fistulas, and also conducts screening of patients for implantation of an artificial left ventricle. Provides proctorial support in various centers for edge-to-edge transcatheter mitral valve surgery. Work experience of more than 10 years. He regularly makes presentations at scientific and practical conferences. He is the author of two Patents and more than 30 publications. In 2015, he took 1st place in the echocardiography competition for young cardiologists under the auspices of the Society of Heart Failure Specialists and the Echocardiography Section of the Russian Society of Cardiology. In 2022, he was awarded a Certificate of Honor from the Federal State Budgetary Institution "NMITSK named after Academician E.I. Chazov" of the Ministry of Health of the Russian Federation.

Under the supervision of E.P. Panchenko, 10 PhD and 2 doctoral dissertations were defended. He is the author of more than 400 publications on the problem of thrombosis diagnosis and antithrombotic therapy in Russian and foreign journals. He is the author of three monographs on the treatment of arterial and venous thrombosis, stroke prevention in patients with atrial fibrillation, as well as sections on antithrombotic therapy, including in the manual of angiology edited by Academician of the Russian Academy of Sciences A.V. Pokrovsky, in the manual of Cardiovascular Diseases edited by Academician of the Russian Academy of Sciences E.I. Chazov and others. Member of the author's team for the development of National recommendations for the diagnosis and treatment of acute coronary syndromes, atrial fibrillation, venous thromboembolic complications, stable manifestations of atherothrombosis, and chronic coronary heart disease. He is the head of the educational School for doctors on the problem of thrombosis and antithrombotic therapy, organized under the auspices of the National Society for Atherothrombosis and the Russian Society of Cardiology.
